Home Health Registered Nurse - full, part-time, and weekend hours available
Registered Nurses, want to work one-on-one with patients and with other health care providers for a team approach? We are looking for great staff interested in working part or full-time hours. Qualified applicants should be organized, a self-starter and enjoy flexibility and creativity in their day.

  • Assess and evaluate the total needs of the patient.
  • Provide direct care to patients in their home.
  • Provide support and teaching of care to patient, family or other caregivers.
  • Conduct comprehensive physical and environmental assessments.
  • Develop individualized nursing care plan.
  • Coordinate discharge planning with patient, physician, family and caregivers.
  • Coordinate care with physician and other agency staff.
  • Participate in on-call rotation.

Home Health Team Manager - full-time

Home Health Team Manager needed for busy Home Health Agency.  The Home Health Team Manager provides clinical coordination consultation to ensure appropriate care and quality of service. Confers with all disciplines and ancillary services to provide quality care to patients.

Qualifications: Minimum of Bachelor of Science in Nursing Degree with suitable operational experience, in good standing and able to meet all federal and state licensure requirements, five years’ clinical experience with a minimum of three years’ experience in a management or lead clinical role and home health experience preferred.

Licensed Practical Nurse - full and part-time hours available
Looking for LPNs to work part-time to full-time. Enjoy working with a team of health care professionals to give direct nursing care to patients. LPNs provide skilled nursing care and incidental tasks essential to the patient's care at home. Coordinate services with patients, families and caregivers as well as other members of the home health or hospice team. Provide blood pressure clinics at designated facilities.

PPE equipment is provided to employees as required for the position per CDC & OSHA guidelines. All staff are required to wear masks in the office except when in their office/work station. VNA is subject to the CMS vaccination mandate.
